Core Functions
Immediate Access
Walk-in access 24 hours a day, six days a week. Closed 8 am Wednesday to 8 am Thursday, every week.
Connect 2 Care is located at 515 28 3/4 Road, Grand Junction, Building F. Enter through the north-side doors under the red band.
Stabilization Services
Peers and Case Managers provide grounding, coping skills, de-escalation, and short-term stabilization in a calm, trauma-informed environment.
Care Coordination
Case Managers and Peers assist with collateral contacts, initial/follow-up scheduling, and warm handoffs to outpatient, medical, and community resources.
Diversion from Emergency & Law Enforcement Systems
A safe alternative to emergency departments and jail for individuals in behavioral health crisis.
Crisis Assessment
Crisis Response Clinicians are available to complete crisis assessments and disposition planning to determine the safest, least, and restrictive steps.
