Immediate Access

Individuals may be present during operating hours to receive a behavioral health screening, assessment, and connection to appropriate services. Open 24 hours a day, six days a week. Closed Wednesdays 8:00 a.m. – Thursdays 8:00 a.m.

Connect 2 Care is located at 515 28 3/4 Road, Grand Junction, Building F. Enter through the north-side doors under the red band.

Behavioral Health Assessment

Master’s-level clinicians conduct assessments to evaluate an individual’s needs and determine the most appropriate level of care, treatment recommendations, and community supports.

Peer Support & Care Navigation

Peer Support Specialists and Case Managers provide recovery-oriented support, resource navigation, collateral contacts, appointment scheduling, follow-up, and warm handoffs to behavioral health, medical, and community resources.

Connection to Crisis Services

When clinically indicated, individuals are connected with Health Solutions West’s Mobile Crisis Response team or other appropriate emergency services for further evaluation, intervention, and coordination of the most appropriate next steps.
